What is IVF?
In vitro fertilization (IVF) is an assisted reproductive technology where eggs are retrieved from the ovaries and fertilized with sperm in a laboratory. The resulting embryos are then transferred to the uterus.
How It Works
Ovarian Stimulation
Injectable medications stimulate the ovaries to produce multiple eggs over 8–14 days.
Egg Retrieval
Eggs are collected from the ovaries using ultrasound-guided aspiration under light sedation.
Fertilization
Eggs and sperm are combined in the lab. ICSI may be used to inject sperm directly into eggs.
Embryo Culture
Embryos develop in the lab for 3–5 days. Genetic testing (PGT) can be done at the blastocyst stage.
Embryo Transfer
One or two embryos are placed into the uterus through a thin catheter. Remaining embryos can be frozen.
Who Is It For?
IVF may be recommended for blocked fallopian tubes, severe male factor infertility, diminished ovarian reserve, endometriosis, unexplained infertility, or after failed IUI cycles.
Who It May Not Be Right For
IVF may not be the best first option for couples with mild infertility who haven't tried less invasive treatments, patients with untreated uterine abnormalities that would prevent implantation, or individuals who are not prepared for the physical and emotional demands of the process.
Success Factors
Success rates depend primarily on age, egg quality, embryo quality, uterine health, and underlying diagnosis. Younger patients generally have higher success rates.
Timeline
A typical IVF cycle takes 4–6 weeks from the start of medications to embryo transfer, with a pregnancy test about 10 days after transfer.
Risks & Side Effects
- Ovarian hyperstimulation syndrome (OHSS) in 1-5% of cycles
- Multiple pregnancy if more than one embryo is transferred
- Ectopic pregnancy (rare)
- Egg retrieval complications (bleeding, infection — very rare)
- Emotional and psychological stress
- No guarantee of success — many patients need multiple cycles
Insurance Reality
As of 2026, 13 states have full fertility insurance mandates and 8 have partial coverage. However, even in mandate states, coverage varies by employer size and plan type. Many plans cap coverage at 3-4 cycles or impose lifetime dollar limits. Always call your insurer directly — don't rely on your employer's summary.
Alternatives to Consider
IUI
Less invasive, lower cost ($1,200-$4,000 vs $15,000-$30,000), but significantly lower success rates (10-20% vs 40-50% per cycle)
Mini IVF
Uses lower medication doses, costs $5,000-$14,000, produces fewer eggs but may be gentler on the body
Natural Cycle IVF
No stimulation medications, lowest cost ($3,500-$8,000), but only retrieves one egg per cycle
Frequently Asked Questions
How many IVF cycles does the average person need?
Most patients need 1-3 cycles. About 50% of patients under 35 succeed on the first cycle. Cumulative success rates after 3 cycles are approximately 70-80% for patients under 38.
Does IVF hurt?
Daily injections cause mild discomfort. The egg retrieval is done under sedation and most women describe it as painless during the procedure, with cramping afterward. The embryo transfer is similar to a Pap smear.
Can I work during an IVF cycle?
Most women continue working throughout the cycle. You may need flexibility for monitoring appointments (early mornings) and 1-2 days off for egg retrieval.
What is the age cutoff for IVF?
There is no hard cutoff, but success rates decline significantly after 40 with own eggs. Many clinics recommend donor eggs for patients over 42-43. Talk to your doctor about realistic expectations for your age.
