IUI (Intrauterine Insemination)

What is IUI?
Intrauterine insemination (IUI) is a fertility treatment where washed and concentrated sperm is placed directly into the uterus around the time of ovulation, shortening the journey sperm must travel.
How It Works
Ovulation Monitoring
Ultrasound and bloodwork track follicle development. Medications may be used to stimulate ovulation.
Sperm Preparation
A semen sample is washed and concentrated to isolate the most motile sperm.
Insemination
Concentrated sperm is inserted into the uterus through a thin catheter — a quick, minimally uncomfortable procedure.
Waiting Period
A pregnancy test is taken approximately two weeks after the procedure.
Who Is It For?
IUI is often a first-line treatment for mild male factor infertility, cervical factor infertility, unexplained infertility, or when using donor sperm. It's less invasive and more affordable than IVF.
Who It May Not Be Right For
IUI is not recommended for patients with blocked fallopian tubes, severe male factor infertility (very low sperm count or motility), advanced endometriosis, or women over 40 with diminished ovarian reserve where IVF would be more time-efficient.
Success Factors
Success rates per cycle are typically 10–20% depending on age, sperm quality, and whether fertility medications are used. Multiple cycles are often needed.
Timeline
Each IUI cycle aligns with your menstrual cycle, typically taking 2–4 weeks from monitoring to insemination.
Risks & Side Effects
- Multiple pregnancy when used with fertility medications (10-20% twin rate with injectables)
- Ovarian hyperstimulation from fertility drugs
- Very small risk of infection from the procedure
- Emotional stress from repeated low-probability cycles
Insurance Reality
IUI is more commonly covered by insurance than IVF, even in states without fertility mandates. Many plans cover diagnostic testing and ovulation induction. However, the sperm washing and insemination procedure itself may not be covered. Donor sperm costs are rarely covered.
Alternatives to Consider
Timed intercourse with medications
Even less invasive — oral medications like Clomid or Letrozole with timed natural conception. Lower cost but also lower success rates than IUI.
IVF
2-3x higher success rates per cycle but significantly more expensive and invasive. Usually recommended after 3-4 failed IUI attempts.
Frequently Asked Questions
How many IUI cycles should I try before moving to IVF?
Most reproductive endocrinologists recommend 3-4 IUI cycles. If you haven't conceived after 3-4 attempts, the likelihood of IUI success drops significantly, and IVF becomes more cost-effective.
Is IUI painful?
Most women describe it as similar to a Pap smear — mildly uncomfortable but not painful. The procedure takes about 5 minutes and requires no anesthesia.
Can I use IUI with donor sperm?
Yes. IUI is one of the most common methods for conception with donor sperm. Single women and same-sex female couples frequently use IUI with donor sperm as a first-line treatment.
