ICSI (Intracytoplasmic Sperm Injection)

What is ICSI?
Intracytoplasmic sperm injection (ICSI) is a specialized IVF technique where a single sperm is injected directly into the center of an egg using a microscopic needle, bypassing the need for sperm to penetrate the egg naturally.
How It Works
Egg Retrieval
Eggs are retrieved through the standard IVF process.
Sperm Selection
An embryologist selects the best individual sperm under high magnification.
Microinjection
A single sperm is injected directly into each mature egg using a glass micropipette.
Embryo Development
Fertilized eggs are monitored for normal development before transfer or freezing.
Who Is It For?
ICSI is recommended for severe male factor infertility (low count, poor motility, abnormal morphology), previous IVF fertilization failure, surgically retrieved sperm, or when using frozen sperm.
Who It May Not Be Right For
ICSI may not be necessary when semen parameters are normal and there is no previous fertilization failure. Some clinics routinely use ICSI on all cases, but evidence does not support a benefit for non-male-factor infertility.
Success Factors
ICSI fertilizes about 70–80% of injected eggs. The overall IVF success rate with ICSI is comparable to conventional IVF for appropriate candidates.
Timeline
ICSI is performed on the same day as egg retrieval as part of the IVF cycle.
Risks & Side Effects
- Very slightly increased risk of certain genetic conditions compared to natural conception (though absolute risk remains low)
- Same risks as standard IVF (since ICSI is part of an IVF cycle)
- Additional cost on top of IVF
Insurance Reality
ICSI is usually covered as part of an IVF cycle when medically indicated (severe male factor). Some plans cover IVF but exclude ICSI as an add-on. The additional $1,500-$2,500 cost may or may not be included in your IVF coverage.
Alternatives to Consider
Conventional IVF insemination
Eggs and sperm are placed together in a dish and fertilization happens naturally — works well when sperm parameters are normal
Frequently Asked Questions
Is ICSI always necessary?
No. ICSI is specifically indicated for severe male factor infertility. For couples with normal sperm, conventional insemination works equally well. Ask your clinic why they recommend ICSI for your case.
Does ICSI increase IVF success rates?
ICSI increases fertilization rates when sperm quality is poor. However, for normal sperm, ICSI does not improve overall pregnancy or live birth rates.
